Professionals and Referrals

A clear referral route for professionals and families.

We welcome enquiries from social workers, commissioners, discharge teams, healthcare professionals, advocates, families and individuals arranging their own support. An initial referral does not commit either party; it gives us enough information to decide whether a full assessment is appropriate.

Helpful information at referral stage

• The person’s age, location and preferred communication method.
• The type and frequency of support being considered.
• Current risks, mobility needs and relevant equipment.
• Any personal-care, medication or behavioural-support requirements.
• Known safeguarding concerns or significant recent incidents.
• Funding or commissioning arrangements, if already confirmed.
• The person’s outcomes, preferences and what is important to them.

Please do not send full medical records or unnecessary sensitive information through the website form. We can arrange an appropriate secure route if detailed information is required.

What happens next

1. Initial review: we check whether the referral appears to fit our service scope and capacity.
2. Assessment: where appropriate, we gather fuller information and involve the person and relevant professionals.
3. Decision: we confirm whether we can safely meet the assessed needs and what staffing or resources would be required.
4. Planning: if accepted, we agree the care or support plan, start date, communication arrangements and review process.

Regulatory fit

We assess referrals against our current CQC registration, staff competence, operational capacity and the specific risks involved. We will not present NCRYSTAL as able to deliver a regulated activity that falls outside our registration.
