Complex care support
Complex Care
Skilled, person-centred support for adults with higher, changing or multiple care needs, planned around safety, dignity and independence.
Complex care built around the individual
NCRYSTAL Health provides person-centred complex care support for adults whose needs are higher, changing or involve several areas of daily living. Every package begins with a detailed assessment so that support is safe, proportionate and built around the person’s routines, preferences, outcomes and risks.
Support we can provide
- Personal care and support with daily living
- Mobility support, transfers and hoisting where assessed
- Medication support within agreed care plans and staff competency
- Continence support and skin-integrity awareness
- Support for adults living with physical disabilities, dementia or sensory impairments
- One-to-one or higher-intensity support where commissioned and assessed
- Waking-night, sleep-in and 24/7 support arrangements where required
- Support with nutrition, hydration, routines, appointments and community access
Safe, skilled delivery
Complex care is only accepted where NCRYSTAL can safely meet the assessed needs. Staff are selected, trained and competency-checked for the individual package, with clear care plans, risk assessments, escalation routes and regular review.
Where a person requires a clinical task that must be delegated by an appropriate healthcare professional, it will only be undertaken when the task can be lawfully delegated, the care plan is clear and the staff member has been trained and assessed as competent. NCRYSTAL does not present nursing or medical treatment as part of its current CQC Personal Care registration.
Working with families and professionals
We work closely with individuals, relatives, social workers, commissioners and healthcare professionals to maintain continuity, respond to changes and keep support focused on safety, dignity and independence.
