Community Support

Practical support and companionship that helps people stay active, included and connected to what matters.

Practical support for a more connected everyday life.

Community support can help people maintain routines, confidence, relationships and access to the places and activities that matter to them. It is planned around the person’s goals rather than a fixed list of tasks.

Support may include

  • Attending appointments and planned activities.
  • Shopping, errands and practical everyday tasks.
  • Building confidence using local services and public spaces.
  • Companionship and support to reduce social isolation.
  • Support with hobbies, education, volunteering or meaningful routines.
  • Travel support where this is agreed and safely planned.

Person-centred and risk aware

We balance independence with proportionate risk management. Where a person needs support with communication, mobility, behaviour, medication or personal care while accessing the community, this is considered as part of the assessment and care plan.

Working with the wider network

With appropriate consent, we can coordinate with family members, social workers, healthcare teams and other professionals so that community support contributes to the person’s wider outcomes rather than operating in isolation.
